Registration Information: Any and all future clinic dates will be posted here. Future clinics will be based on the need in our community. 

January 18th, 2022 Vaccine Clinic (3:30-5:30pm):

A second dose clinic for those that get their first dose on 1/18/22 will be held on 2/8/22. More information and a registration link for second dose appointments will be sent via email after the first dose clinic.

A second dose clinic for those that get their first dose on 1/20/22 will be held on 2/10/22. More information and a registration link for second dose appointments will be sent via email after the first dose clinic.

Insurance information: the only information required on the health insurance information section is the type of insurance you have (private, public, Medicare, etc.).  Once you select the type of insurance, go to the bottom of this page and click ‘Save and Continue.’

Review your Details/Summary Page: You must scroll to the bottom of this page and click on ‘Save and Continue’ or your appointment will not be saved. A confirmation email will be sent to those who successfully register.

On the day of your appointment: please arrive on time and bring your vaccine card if you have one. Masks are required in our clinics and one will be provided to you upon entry. The entire appointment time should take around 20-25 minutes, which includes a wait time of 15 minutes after being vaccinated to monitor for any adverse reactions. It is also important to eat/drink before your appointment.

Adults ages 18+ can get any of the COVID-19 vaccines authorized in the United States for a booster (Moderna/Pfizer are preferred in most situations)

Moderna
Those ages 18 and older
At least 5 months after completing your primary COVID-19 vaccination series
Any of the COVID-19 vaccines authorized in the United States for a booster (Moderna/Pfizer are preferred in most situations) 
Johnson & Johnson/Janssen
Those ages 18 and older
At least 2 months after completing your primary COVID-19 vaccination
Any of the COVID-19 vaccines authorized in the United States for a booster (Moderna/Pfizer are preferred in most situations)
